Patient resources

The practice is in network with 12 dental plans, all listed on this page. Our staff review your insurance details before your visit and the practice submits the claim to your in-network plan, which then decides what you owe. Without one of these plans you pay the practice directly, and payment plans are available.

Not on the list, or no dental insurance? You pay the practice directly, and the fee for your case is given to you in writing at your consultation. The practice does not accept Medicaid.

Before your visit

Having these ready when you book lets staff review your plan before you arrive.

  1. Your insurance card

Front and back. The member ID, group number and the plan phone number are all on it.

  2. The policyholder

If the plan is through a parent, spouse or employer, the policyholder name and date of birth.

  3. Any second plan

If you have dental cover from two sources, bring both cards so the claim goes to the right plan first.

Questions about insurance

My plan is on the list. What happens next?
Give us your insurance details when you book. Our staff review them by hand before your visit, and after your treatment the practice submits the claim to your plan. Your plan then decides what you owe, so we do not promise a figure in advance.
My plan is not on the list. Can I still be seen?
Yes. You pay the practice directly, and the fee for your case is given to you in writing at your consultation, before anything is booked. Some plans reimburse members for care outside their network; ask your plan what it needs from you.
Does being in network mean my surgery is paid for?
Not on its own. In network means the practice has an agreement with the plan and submits the claim for you. What the plan pays depends on your own policy, your deductible and your remaining benefits, and only your plan can decide that.
Is Medicaid one of the plans?
No. The practice is not in network with Medicaid, including Florida Medicaid, and does not accept it.
